I am not a doctor, but I am a former science teacher and current nurse so I can take some educated guesses. My best guess, based on the photos is that the left photo is the oldest and the top right is the most recent. Hard to tell but I feel like that might be fungal growth on the outside of his hand. The tissue is definitely not healthy and certainly poses a risk of systemic infection. The human body has evolved to function in its default orientation (hands down). If you hold your arm up for a prolonged period, even as little as a few minutes, you'll probably start to notice some tingling. This is because your heart isn't efficiently pumping blood all the way to your extremities and it isn't exchanging gases and various other things the way it's meant to. There's a good chance you've seen someone at some point in your life that has circulatory issues, you can tell because their lower legs are a darker color and sometimes the skin looks a bit scaly. Not quite the same issue because that's caused by blood not returning from the extremity effectively. In this gentleman's case, it's not getting to the extremity effectively. When he lays down horizontally I would suspect the blood flow is more normal, although after all this time I assume it doesn't come back to baseline but must perfuse the tissue enough to keep it alive. Cant tell from the photo but it's very likely that he has tissue death and decay starting as well. If it were more advanced, he would likely have a systemic infection and die without proper treatment. As far as returning his function to normal, I have less insight since that would be a surgical and rehabilitation question. I would assume that he would require a total surgical replacement of his joint in order to put his arm down and likely require surgical intervention for revascularization as well. Although, this would also lead to other medical issues. Again, not a doctor, these are my best guesses, hopefully someone with more knowledge than me can add to this/ correct any inaccuracies.
